Another way to empower renal patients is to help them understand their disease and life situation better. The Diaverum education programme increases patients’ understanding of their situation and how they can take an active role in deciding how to live their life with CKD. The programme, unique in the market, comprises eight learning modules, a patient handbook, an e-learning tool and a multi-channel tool box – all designed to empower patients to live a fulfilling life.
Giving the patient a voice
Diaverum aims to provide the best renal care possible. That is why we encourage our patients to give feedback across our entire chain of services. Valuing the patient’s voice is key to getting a better understanding of their priorities and concerns. Our yearly survey, in partnership with the Integrated Care University, is asking all patients to share their thoughts and experiences. The survey is short and precise, yet covers the topics most important to our patients, including trust, involvement, understanding the treatment (nutrition and diet), waiting time, and care improvement.
