CKD is an escalating, global challenge. It has multiple causes, including many linked to unhealthy lifestyles. Moreover, it adds to the unsustainable growth in expenditure and disease burden affecting national healthcare systems & patients around the world.

To tackle this, digital innovation is key – it has the potential to unlock an unprecedented level of personalised, standardised, efficient and high-quality care.

In the future, we see physical clinics and digital platforms combining to improve renal care. This will include AI predictive analytics and mobile apps contributing to the delivery of efficient and personalised care, adjusted to each patient’s needs.

Digital is therefore reshaping the future of our industry and we are determined to play a leading part in this.

"We are not building digital solutions around dialysis machines; we are building digital solutions around our patients."
Our digital transformation

In the last 3 years, Diaverum has embarked on an ambitious journey to convert 30 years of dialysis know-how into one digital platform that connects with clinics across the world to ensure the highest standards of care for patients, the most advanced support for our clinical teams and the lowest cost for payors.

Our digital platform’s solutions and applications are developed around our patients’ needs, improving treatment effectiveness, efficiency and medical outcomes worldwide, while also empowering them to contribute to their own care and well-being.

This includes human-centric, explainable, and responsible AI solutions to support our physicians in delivering personalised care to our patients. Read more about Diaverum Artificial Intelligence here.

"As part of our digital journey, we have been converting all our know-how into a proprietary digital platform, so we can provide the most efficient, standardised, high-quality and highly scalable ESRD service in the industry."
Our digital solutions and applications

The TGS supports our nursing staff in adhering to standardised clinical workflows, minimising care variability and margin of error. Accessed via a tablet adjacent to the patient’s chair or bedside, the TGS collects and provides information throughout the treatment session, in a paperless environment.

The TGS also connects with the Diaverum Enterprise Resource Planning system which manages stock and financial transactions.

d.CONNECT is a fully automated solution for data collection, encompassing dialysis monitors and weighing scales. It then extracts and stores data, that then automatically populates in the TGS.

Our PGS is an algorithm-based system. In combination with the personalised data retrieved from d.CARE, it advises doctors on optimal medication requirements, based on the needs of each individual patient retrieved from d.CARE.

d.CARE is our renal information management system. It contains data related to clinical management, medical reporting and clinic processes and administration. It drives clinical workflows and medication management.

The d.CARE patient application empowers patients to engage with their healthcare team in a two-way conversation about their treatment, via their smartphone or tablet. It is both iOS and Android-enabled.

Patients use the app to input information about their health, diet, exercise habits and overall quality of life & lifestyle. In turn, the app provides information related to their treatment, and thus actively engages them in their own care and well-being.
